Job Description:
The Licensed Practical Nurse (LPN) provides direct and indirect
home care, following physician orders and care plans.
Responsibilities include administering medications, documenting
patient progress, teaching self-care, and performing
assessments.
Ensures compliance with regulatory standards, maintains patient
confidentiality, and communicates effectively with team members,
clients, and families.
Performs supervisory visits, coordinates medication management, and
supports staff training.
Requires an accredited nursing degree, valid license, CPR
certification, and relevant experience, preferably in home
health.
Strong communication, organizational, and critical thinking skills
are essential.
Work involves flexible hours, including weekends and holidays, with
a pay rate of $29/hour.
