Eligible/Certified Urgent Care PhysicianBase: $250,000RVU:
$9,500RVU Incentive: $10,000 guarantee Year 1; based on production
after Year 1Sign-on Bonus: $50,000 with a 3 year commitmentPaid
Time Off: Vacation accrues; 3 weeks Year 1; 4 weeks at Year 4. Sick
time accrues; 40 hours annually; 6 paid holidays; calendar year
allotment for CME at $3000/ 40 hours prorated if employment begins
other than Jan. 1Benefits: Health, dental long-term disability and
life insurance provided. Short term disability, additional life and
vision available. Retirement plan with match
